Change of Service Moving - Moving within or out of the service area

  • Use this form to change you service address.
  • To help us close your account and (if applicable) open your new account on the correct date, please complete this form.
  • We require at least five business days’ notice before your last planned day of service. We will do a final meter reading within two to five days of your move date.

Self-Declaration Form - Farming and Multi-Residential Unit Premises

  • The electric meter services more than one residence
  • This form to be completed only if customer would like to be billed on the Regulated Price Plan

Self-Declaration Form – 8% Rebate on Electricity Bill

As of January 1, 2017, the Government of Ontario is introducing an 8% rebate under the Ontario Rebate for Electricity Consumers Act, 2016 (“The Act”) equal to the provincial portion of the Harmonized Sales Tax (HST) on the electricity bills of residential, farm, small business and other eligible customers. This 8% rebate will begin to appear on your electricity bills on electricity consumption starting January 1, 2017.

If you previously provided Burlington Hydro Inc. (BHI) with a self-declaration form to be exempted from the Debt Retirement Charge (DRC) or to receive the Ontario Clean Energy Benefit (OCEB), you will automatically be enrolled to receive the 8% rebate. Please note that if your status has changed since you provided these forms, it is your responsibility to notify Burlington Hydro of that change as it may affect your eligibility to receive the 8% rebate.

In order to ensure that eligible commercial customers receive the 8% rebate, a Self-Declaration Form must be completed and returned by commercial customers who meet the criteria for the 8% Rebate. See the letter
